Beneath the city, a community of mutants has fallen victim to a horrifying experimental drug created by the mysterious Dr. Savary. Their only hope lies with Nicole, a woman inexplicably immune to its terrifying effects. When she is abducted, private investigator Roy Bain is hired to bring her back, only to find himself descending into a nightmarish underworld where survival is anything but certain.

Released in the United States as Transmutations, Underworld offers an early glimpse into the dark imagination of Clive Barker, complete with grotesque practical effects, surreal atmosphere and relentless horror.
