As a child, Luther witnesses the disturbing spectacle of a carnival “geek,” a caged alcoholic performer driven to bite the heads off live chickens for liquor. The experience fractures his mind, leading to a series of murders that lands him behind bars for decades. Thirty years later, Luther is released on parole, fully transformed into a near-feral killing machine, communicating only through chicken clucks and armed with razor-sharp metal dentures.
 
Returning to a quiet rural town, Luther resumes his rampage, terrorizing the local population before taking a young mother and her family hostage in an isolated farmhouse. What follows is an escalating siege of sadistic violence, culminating in an ending infamous for its relentless intensity.
