Dyanne Thorne returns as the sadistic and ruthless Ilsa, now running a remote psychiatric clinic in a Latin American dictatorship where political prisoners are secretly detained and tortured. When a young woman has herself committed in order to locate and rescue her imprisoned sister, she quickly discovers the facility is a nightmare of manipulation, cruelty and power games.
 
Among the prisoners is Ilsa’s mysterious favourite captive, played by cult icon Lina Romay, who quietly plots her own survival while navigating the brutal hierarchy inside the asylum. As alliances shift and tensions rise, the inmates begin to challenge the authority of one of exploitation cinema’s most infamous villainesses.
